The work of the Urgent Community Response Service was highlighted on ITV Meridian last week. 

Across Hampshire and Isle of Wight, our community teams have been working closely together to further develop their Community Urgent Care Response (UCR). This is provided by a multi-skilled team to patients in their usual place of residence with an urgent care need. For patients registered with Hampshire and Isle of Wight GP practices, the UCR can support intermediate wrap around care.

Referrals are accepted from South Central Ambulance Service, GP practices, community teams, same day emergency care centres and emergency departments. The service is available for people aged over 18, experiencing a sudden deterioration in their health and wellbeing, and requiring intervention within two hours to stay safely at home. For example, this may be an urgent equipment need, crisis support for end of life care, support following a fall, and reduced function and mobility.

The UCR team can conduct a holistic and clinical assessment in the patient’s home, and ensure they receive rehabilitation or onward referral to other community based services if needed. The UCR team remains in close contact with the patient’s GP practice which remains responsible for medical care needs.
